Office Policies
Your appointment is a time Dr. Pham has set aside to be reserved exclusively for you. If you are more than 10 minutes late, we ask that you call us. We will do our best to accommodate you, however if you are more than 15 minutes late, we reserve the right to reschedule your appointment.
If you should need to cancel or change your scheduled appointment, please give us at LEAST 24 HOURS NOTICE so we can offer this time to another patient who is awaiting treatment. Without proper notice from you, we reserve the right to charge a $50.00 broken appointment fee.
We are open Tuesday through Saturday and we offer convenient Saturday and evening appointments, and these are highly desirable times for our busy dental patients. If you miss two or more appointments, we will no longer be able to schedule you for any Saturday or evening appointments.
For your convenience, we accept Cash, check, Visa, MasterCard, American Express, Discover Card and Care Credit.
As a courtesy to you, we will help you process all your insurance claims. We provide our patients with an insurance estimate. This is not a guarantee that your insurance company will pay this amount. Your insurance policy is between you, your employer and your insurance company.
Insurance companies usually process payments within 30-45 days. If they have not paid a claim by 45 days after the date of service, we ask that you contact your insurance company to inquire when payment is expected. Charges that are incurred are your responsibility regardless of what your insurance company pays. If we have not received payment from your insurance company by 60 days after date of service, you are responsible for the full amount due. Balances which are 60 days overdue are subject to a finance charge of 1.5% per month (18% annual).
Any balances of 90+ days past due will be sent to a collection agency and you will be responsible for attorney and court fees that will be incurred. Returned checks are subject to a $50 bank and accounting fee.
We do not file secondary insurance, but we will gladly assist you in filing the secondary claim yourself.
